General Description
The Cornerstone Case Management Expansion will include additional services to Cornerstone Community Centers. This project is located in the Brownsville community of Brooklyn, New York - (NYCHA) Van Dyke Houses Community Center. This expansion consists of a case-management approach that incorporates a Social Worker, a Navigator, and the development of Peer Support Leaders into assigned Cornerstone Community Centers. The Social Worker and Navigator will create connections to other City agencies and providers and act as “expediters” for resources such as health, mental health, employment training, housing, food, etc., to help young adult Participants (ages 18-21) with issues identified in their Individual Service Plans. The Navigator will work with the Mayor’s Office of Criminal Justice’s Office of Neighborhood Safety to coordinate efforts with Cure Violence providers and Mayor’s Action Plan community engagement efforts to help create a network of safety and accountability for the young adults while connecting them to a neighborhood improvement strategy that empowers them to be a part of positive change.
Project requires staff to address the areas listed in this scope of work, including positions but not limited to:

Other Duties
Evening and Weekend Hours are required as part of regular work schedule
Qualifications
Social Worker:
A. The Social Worker will lead the creation and implementation of the case-management model at the Cornerstone Program (“the Program”). Utilizing a strength-based, youth-centered, and trauma-informed lens, the Social Worker shall provide direct services ranging from prevention of high-risk activities to crisis intervention for Participants, their families, and the immediate community. Specifically, the Social Worker shall perform functions including but not limited to: i. Initiate contact and establish trust amongst identified high-risk Participants currently in the Cornerstone program;

- ii. Identify Participants who have experienced or are experiencing trauma and establish weekly Healing Circles for them and high-risk groups;
- iii. Holistically assess each Participant’s strengths and needs and develop an Individual Service Plan (“ISP”). The ISP shall include, at a minimum, goals for education, employment, health, and life skills for the Participant;
- iv. Work with the Navigator to implement the ISPs;
- v. Work with Participants to foster positive relationships with peers and adults;
- vi. Support Participants’ family needs and provide/refer those in need to additional resources and services; and
- vii. Serve as a supportive resource to other on-site staff at the Cornerstone community center.

B. The Social Worker must have the following qualifications: i. Master’s Degree in Social Work (LMSW) from an accredited college; and

- ii. Two years of experience in community work or community-centered activities related to the duties described in Section 1(a).
